The possibility of an underlying genetic defect in lipoprotein metabolism was also raised to account for hypertriglyceridemia in some diabetics, although the data on ketosis do not support this (23, 24, 27). The mechanism by which hyperlipidemia leads to AP is not well established, but may be related to accumulation of noxious free fatty acids and lysolecithins in and around the pancreatic acinar cells by the action of pancreatic lipase and phospholipase A2 (28–30).
