Abstract

The world literature on chylothorax is reviewed and the embryology, anatomy, and physiology of the thoracic duct system are detailed. The various types of chylothorax are classified and the diagnostic methods and treatment for each type described. The review concludes with a summary of the modern concepts of treatment of chylothorax.

      The antenatal course is variable with spontaneous regression occurring in approximately 20% of primary fetal hydrothorax cases.5 When congenital pleural effusions progressively enlarge and compromise fetal hemodynamics and cardiac function, secondary hydrops fetalis may develop.6,7 The presence of hydrops is an important risk factor for poor prognosis.5

      Because chylomicrons comprise mainly LCT, and LCT is the major component of dietary fat or fat-containing enteral formula, limiting or deleting enteral LCT administration is effective management for postoperative chyle leak. Nevertheless, use of TPN [20,21] or EN containing middle-chain triglycerides (MCT) [19,22] has been reported as a conservative management for postoperative chyle leak. The low-fat elemental formula, ELENTAL® (Ajimonoto Pharmaceuticals CO., LTD., Tokyo, Japan) was recently recommended for the management of postoperative chyle leak [23].
