Kokain-body-packing

Abstract

Introduction. Body-packers or "mules" are drug smugglers who swallow cocaine-filled condoms in order to conceal them during air travel. Body pushers hide drug packages in the rectum or vagina. In a cooperative effort between the Frankfurt Airport Clinic and the GIZ-Nord (Goettingen University poison control center), we performed a retrospective study and developed an algorithm for the problem of "rupture of a cocaine-filled condom in a body-packer."

Methods. In a retrospective analysis, the data of all cocaine bodypackers and body pushers who were identified at Frankfurt International Airport from 1985 to 2001 were evaluated.Temporal development, demographic data, and surgical aspects were of special interest.

Results. From 1985 to 2001 a total of 280 body pushers and 2880 body-packers were identified: 63 "mules" (2.2%) developed symptoms of severe cocaine intoxication following rupture of a condom. Emergency laparotomy was performed on 20 patients (i.e., 32% of all symptomatic body-packers) and the condoms were removed, while 43 body-packers (68%) died before surgical therapy could be initiated. All operated patients survived.

Conclusion. Severe cocaine intoxication is life threatening. Patients die from complications caused by generalized vasoconstriction. If the reason for severe cocaine intoxication is the rupture of a cocainefilled condom,the only possible therapy consists of immediate laparotomy for removal of the condoms.

Zusammenfassung

Einleitung. Unter "body-packers" oder "mules" versteht man Drogenkuriere, die mit Kokain gefüllte Kondome verschlucken, um diese bei einer Flugreise zu schmuggeln. "Body pushers" führen sich die mit Drogen gefüllten Behältnisse von rektal oder vaginal ein. Im Rahmen einer Kooperation zwischen dem Giftinformationszentrum-Nord und der Airport-Klinik am Frankfurter Flughafen wurde mittels einer retrospektiven Analyse ein Therapiealgorithmus bezüglich der Problematik "Kondomruptur bei Kokain-body-packers" erarbeitet.

Methoden. Die Daten aller im Zeitraum von 06/1985–06/2001 am Frankfurter Flughafen detektierten "body-packer" und "body pusher" wurden hinsichtlich zeitlicher Entwicklung,demographischer Daten und medizinisch-chirurgischer Aspekte ausgewertet.

Ergebnisse. Von 1985–2001 wurden auf dem Frankfurter Flughafen insgesamt 280 "body pusher" und 2.880 "body-packer" detektiert; 63 "mules" (2,2%) wurden durch eine schwere Kokainintoxikation in Folge einer Kondomruptur symptomatisch. Bei 20 Patienten (32% der symptomatischen "body-packer") wurde eine notfallmäßige Laparotomie mit Entfernung der Kondome durchgeführt; 43 symptomatische "body-packer" (68%) verstarben, bevor sie einer chirurgischen Therapie zugeführt werden konnten. Alle 20 laparotomierten Patienten überlebten.

Schlussfolgerungen. Die schwere Kokainintoxikation ist lebensbedrohlich. Ohne adäquate Therapie versterben die Patienten an den Folgen einer generalisierten Vasokonstriktion. Liegt der Intoxikation ein geplatztes Kokainkondom bei einem "body-packer" zugrunde, so besteht die einzige kausale Therapieoption in der notfallmäßigen Laparotomie mit Entfernung der Kondome.
