A 41-year-old man presented with a 3-week history of fever, night sweats, retching, abdominal pain and excessive weight loss with no effect of oral antibiotics and omeprazole. He had insignificant medical history, never used intravenous drugs and was a teetotaller. He was pyrexial and tachycardic, with normal abdominal examination.
